Cervical Spondylosis

Cervical spondylosis(Arthritis of Neck) is the degeneration of the joints in the neck. More than 85% of people over age 60 are affected. Neck pain arising from the cervical spine is typically precipitated by neck movements and may be accompanied by focal spine tenderness and limitation of motion. Cervical spine trauma, disk disease, or spondylosis may be asymptomatic or painful and can produce a myelopathy, radiculopathy, or both. The nerve roots most commonly affected are C6and C7.Pain may be neurogenic, when the spinal cord or nerve roots are compressed. It is the most common cause of spinal cord dysfunction in patients older than 55 years.

Causative factors for cervical spondylosis

  • As you get older, the discs tend to dry out and become susceptible to tears. Body will also try to compensate for the wearing of the joints by producing small lumps of extra bone to better support of the neck and stiffness of the spine. These lumps of extra bone are known as bone spurs or osteophytes.
  • Osteophytes can cause the spine to become too rigid, leading to stiffness and neck pain. The changes in bone structure can also squash nearby nerves and the spinal cord. This tends to be more common in older people. A slipped disc, also called a prolapsed or herniated disc, occurs when one of the discs of the spine is ruptured (splits) and the soft inside spills out.
  • If this soft material presses against a nerve in the neck, it can cause severe pain radiating to the arm (cervical radiculopathy), and can occasionally result in compression of the spinal cord (cervical myelopathy).
  • Slipped discs are generally seen in younger people and are not as common as the process of osteophyte formation described above.

Symptoms

Pain from cervical spondylosis can be mild to severe. It is sometimes worsened by looking up or down for a long time, or with activities such as driving or reading a book. It also feels better with rest or lying down.

   Additional symptoms include:

  • Neck pain and stiffness
  • Numbness and weakness in arms, hands, and fingers
  • Trouble walking, loss of balance, or weakness in hands or legs
  • Muscle spasms in neck and shoulders
  • Headaches
  • Grinding and crackling sound/feeling in neck with movement

How to diagnose cervical spondylosis?

  • During the examination, doctor will check the range of motion in the neck. To find out if there's pressure on the spinal nerves or spinal cord, doctor will test the reflexes and check the strength of the muscles and to watch the walk to see if spinal compression is affecting the gait.
  • Neck X-ray. An X-ray may show abnormalities, such as bone spurs, that indicate cervical spondylosis.
  • MRI uses a magnetic field and radio waves and can produce detailed, cross-sectional images of both bone and soft tissues. This can help pinpoint areas where nerves may be getting pinched.
